Community Care Program (CCP)

State-funded support for seniors aged 60+ to remain independent and safe in their own homes.

Safe Life Home Health Care proudly participates in the Illinois Department on Aging’s Community Care Program (CCP)—offering homemaker services at no cost for qualifying seniors. CCP is designed to delay or prevent the need for nursing home placement by supporting daily living needs at home.

Who Qualifies?

To be eligible for CCP services through Safe Life, individuals must:
  • Be 60 years of age or older
  • Be a legal resident of Illinois
  • Be a U.S. citizen or qualified non-citizen
Safe Life assists all applicants in working with their local Care Coordination Unit (CCU) to complete the eligibility and application process.

Services Offered Under CCP

Homemaker Services

  • Assistance with bathing, grooming, dressing
  • Medication reminders
  • Light housekeeping (laundry, kitchen, sweeping)
  • Meal preparation
  • Grocery shopping and errands
  • Escort to medical appointments or essential travel

Live-In Assistance

  • 24-hour presence for safety and supervision
  • Overnight care and companionship
  • Support with toileting, ambulation, and personal routines
  • Ideal for seniors living alone or with progressive care needs

Emergency Home Response Services (EHRS)

  • 24/7 emergency alert pendant system
  • Fast connection to emergency responders
  • Ideal for individuals living alone or at fall risk

Scheduling & Care Planning

  • Number of service hours determined by DON assessment
  • Care typically ranges from 6 to 40 hours/week
  • Schedule is flexible and customized to family routines
  • Re-evaluation occurs every 6 or 12 months or upon request

Financial Structure

  • Fully state-funded for eligible individuals (no out-of-pocket costs)
  • Sliding scale for those with moderate income
  • Income limits adjusted annually by Illinois Department on Aging
  • Safe Life supports you through all steps: screening, application, and ongoing benefit coordination

Coordination & Support

  • Safe Life coordinates with your local Care Coordination Unit (CCU)
  • We handle referrals, re-assessments, scheduling, and compliance
