A nurse or therapist visits your home to assess your health status, mobility, daily activities, family support, home environment, and any emotional or social needs.
We involve patients and families in establishing clear, meaningful goals—whether it’s regaining strength post-surgery, managing chronic conditions, or maintaining independence.
Based on the assessment, our RN creates a physician supervised care plan. Then, your team—nurse, PT, OT, social worker, hospice staff—is assigned and trained on the plan.
Care plans reflect your language, culture, routines, dietary preferences, and personal beliefs to ensure respectful and relevant care.
