Role Overview

Lead a home‑health nursing caseload, complete OASIS‑E assessments, coordinate care across disciplines, and educate patients/caregivers to prevent avoidable hospitalizations. You’ll collaborate closely with physicians and therapy teams and document timely in WellSky.

Key Responsibilities

Complete SOC/ROC/Recert/DC visits and OASIS‑E documentation with accuracy and timeliness.
Develop and update individualized Plans of Care; obtain and carry out physician orders.
Deliver skilled interventions (wound care, medication reconciliation, disease‑specific teaching, safety planning).
Coordinate services with PT/OT/ST/MSW/HHA; lead/participate in case conferences.
Monitor early‑warning signs and initiate interventions that reduce rehospitalizations.

Ensure documentation meets Medicare CoPs/PDGM and payer requirements, close documentation within agency timeframes.

Communicate promptly with providers, patients, and families; maintain HIPAA compliance.

Qualifications

Active Illinois RN license; BLS/CPR.

1+ year home‑health RN experience preferred (new to HH considered with strong acute background).

Proficiency with EMR; WellSky (Kinnser) preferred.

Excellent clinical assessment, communication, organization, and time‑management skills.

Valid driver’s license, auto insurance, and reliable transportation.

Schedule & Pay

Full‑time or Part‑time with field travel; competitive hourly/visit rates based on experience.
