We do not sell your personal information. We may share information in the following limited circumstances:

4 HIPAA & protected health information

Your Protected Health Information (PHI) — including clinical records, treatment history, and billing information — is governed by HIPAA and the HITECH Act. Safe Life Home Health Care, LLC maintains comprehensive administrative, physical, and technical safeguards to protect PHI from unauthorized access, use, or disclosure.

Patients and authorized representatives receive our full Notice of Privacy Practices (NPP) at the start of care. The NPP details all permitted uses and disclosures of PHI and is available upon request at any time.

For any concerns regarding your PHI, you may also file a complaint directly with the U.S. Department of Health & Human Services Office for Civil Rights (OCR) at hhs.gov/ocr. We will not retaliate against you for filing a complaint.

We retain patient records in accordance with Illinois state law and CMS Conditions of Participation — generally a minimum of five (5) years from the date of discharge, or three (3) years after a minor patient reaches age 18, whichever is longer.

Website and marketing data is retained for no longer than necessary to fulfill the purpose for which it was collected. We implement industry-standard security measures including encrypted data transmission (TLS/SSL), access controls, employee training, and regular security assessments to protect all information we hold.

 

7 Your rights

Request corrections to inaccurate or incomplete information in your health record.

8 Children's privacy

Our website is not directed to children under the age of 13. We do not knowingly collect personal information from children under 13 without verifiable parental consent. If you believe a child has provided us personal information, please contact our Privacy Officer and we will promptly delete it.

For minors who are patients, all personal and health information is collected under the direction of their legal guardian and is governed by applicable state and federal law.
