Safe Life Home Health Care proudly participates in the Illinois Department on Aging’s Community Care Program (CCP)—offering homemaker services at no cost for qualifying seniors. CCP is designed to delay or prevent the need for nursing home placement by supporting daily living needs at home.

Who Qualifies?

To be eligible for CCP services through Safe Life, individuals must:
Safe Life assists all applicants in working with their local Care Coordination Unit (CCU) to complete the eligibility and application process.
