Answers below are tailored specifically to Safe Life Home Health Care’s participation in the Illinois Community Care Program (CCP), based on up-to-date standards and what we provide.

What is the Illinois Community Care Program (CCP)?
The CCP is a state-funded program offered through the Illinois Department on Aging to help older adults (60+) remain independent and safe in their own homes. It provides free or low-cost homemaker services to qualifying seniors.

To qualify, individuals must:

  •   Be 60 years or older
  •   Be a legal resident of Illinois
  •   Be a U.S. citizen or qualified non-citizen
  •   Have non-exempt assets under $17,500 (single individual)
  •   Be determined through the DON (Determination of Need) assessment to require in-home services
  •   Not currently enrolled in Medicaid HCBS Waiver programs

We provide:

  •   Homemaker and personal care services
  •   Grocery shopping and meal prep
  •   Medication reminders
  •   Light housekeeping and laundry
  •   Accompaniment to medical appointments
  •   Live-in care where needed and appropriate
  •   24/7 Emergency Home Response System (EHRS)
For those who qualify, services are covered fully by the state with no out-of-pocket cost. In cases of moderate income, a small co-payment may apply, depending on state guidelines.
No physician order is required. Eligibility is determined through a state-certified Care Coordination Unit (CCU), which performs the DON assessment and submits paperwork on your behalf.

  • Guide you through the application and enrollment process
The frequency of services is based on the DON score and resulting care plan. Most clients receive between 6 and 40 hours of support weekly. Live-in care and emergency response systems are coordinated based on individual needs.
Yes. Once your application is approved and your care plan established, you may choose from a list of licensed CCP providers. Simply select Safe Life Home Health Care during this step or notify your CCU caseworker.
Yes. We offer services in multiple languages depending on staff availability. Please let us know your preferred language during the intake process.
