General Home Health Care

What is home health care?

It is physician ordered, in home medical care provided by licensed nurses, therapists, and social workers. Services may include wound care, medication management, physical therapy, and chronic disease education—all delivered where you live.

Medicare requires that you be homebound, under a doctor’s care, and in need of skilled nursing or therapy on an intermittent basis. Medicaid, Medicare Advantage, and many private insurers follow similar criteria.

Ask your physician to fax a referral to 630‑406‑9342 or call us directly. We verify benefits, schedule an on‑site assessment, and create a personalized care plan—often within 48 hours.

Yes. Medicare Part A or B covers 100 % of approved charges. Medicaid and most private insurers also cover skilled home health visits at little or no cost to you.
Frequency is based on your care plan and can range from one to three visits per week. We adjust the schedule as your condition improves or changes.

Private Duty (Non Medical) Home Care

What is private duty home care?
Non medical assistance with daily activities—bathing, dressing, meal prep, companionship, and respite—that helps clients live safely and comfortably at home.

Within 24–48 hours after a free consultation, caregiver match, and finalized schedule.

Primarily private pay, but many clients use long term care insurance, VA Aid & Attendance, or certain Medicaid waivers.

Illinois Community Care Program (CCP)

What is the CCP?
A state funded program that provides no cost homemaker services to Illinois residents aged 60 + who meet financial and functional eligibility requirements.

You must be 60 +, an Illinois resident, have non exempt assets under $17,500, and receive a Determination of Need (DON) score indicating in home support is required.

Homemaker assistance, grocery shopping, light housekeeping, medication reminders, appointment escorts, and an optional 24/7 emergency response system.
Call our CCP intake line at 855 723 3440. We’ll connect you with a local Care Coordination Unit to schedule your DON assessment and select Safe Life as your provider.

Hospice & Palliative Care

What is hospice care?
Comfort focused care for individuals with a life expectancy of six months or less, covering pain management, medical equipment, and emotional support.
Palliative care supports anyone with a serious illness, at any stage, alongside curative treatments. It focuses on symptom relief and quality of life.

Yes—Medicare, Medicaid, and most private insurers cover hospice 100 %, including medications and equipment related to the terminal diagnosis.

A hospice medical director, registered nurses, hospice aides, social workers, spiritual counselors, and trained volunteers—all coordinated under one care plan.
We provide caregiver training, 24/7 nurse access, crisis support, and bereavement counseling for up to 13 months after a loved one’s passing.
