Getting Started

Admission Call

After we receive your physician’s referral or your self‑referral, a Care Coordinator will call to:

  • Confirm your insurance benefits and any copays (most patients pay $0 under Medicare).
  • Schedule your first in‑home assessment.

First Home Visit

A registered nurse or therapist will:
  • Complete a head‑to‑toe assessment.
  • Review medications and reconcile any discrepancies.
  • Work with you to set recovery goals.
  • Explain your customized Care Plan and visit schedule.

Your plan is reviewed weekly by our multidisciplinary team and adjusted as needed.

Telehealth & Remote Patient Monitoring

Prefer virtual check‑ins between in‑home visits? Our HIPAA‑secure Telehealth app lets you:

  • Video‑chat with your nurse.
  • Upload blood pressure, glucose, or weight readings from Bluetooth devices.
  • Receive real‑time alerts if a reading is outside your safe range.

Medication Management

  • Keep all medications—prescription, over‑the‑counter, and supplements—in one location.
  • Use the Medication Log Sheet (download below) to track doses and side effects.
  • Bring questions to your nurse or pharmacist; never stop a medication without consulting your doctor.

Patient Rights & Responsibilities

  • You have the right to:
  • Receive care that is considerate, respectful, and free from discrimination.
  • Be informed of your diagnosis, treatment, and expected outcomes.
  • Participate in the development of your Care Plan and refuse treatment.

  • Notifying us of any changes in your condition, address, or insurance.

Infection Prevention at Home

  • Wash hands with soap for 20 seconds before and after dressing changes.
  • Keep wound dressings dry and intact.
  • Clean high‑touch surfaces (doorknobs, phones) daily with a disinfectant wipe.
  • Ensure anyone with cold or flu symptoms wears a mask when visiting.

How long will I receive home health services?
Most episodes last 30–60 days, but services continue as long as they are medically necessary and ordered by your physician.
We can arrange rental of walkers, shower chairs, oxygen, or other durable medical equipment through your insurance.

Call 911 first, then notify Safe Life when it’s safe to do so.
