Lead a home‑health nursing caseload, complete OASIS‑E assessments, coordinate care across disciplines, and educate patients/caregivers to prevent avoidable hospitalizations. You’ll collaborate closely with physicians and therapy teams and document timely in WellSky.

Provide in‑home skilled nursing under RN/physician direction: medication administration, wound care, patient teaching, and safety monitoring. Support timely, accurate documentation to ensure continuity of care.

Evaluate mobility and functional deficits in the home, design evidence‑based treatment plans, and restore safe independence. Educate patients/caregivers and coordinate with nursing and rehab colleagues.

Restore safety and independence in ADLs/IADLs by assessing function, adapting the home environment, and training patients/caregivers. Partner closely with PT and nursing to optimize outcomes.
