Role Overview

Restore safety and independence in ADLs/IADLs by assessing function, adapting the home environment, and training patients/caregivers. Partner closely with PT and nursing to optimize outcomes.

Key Responsibilities

Conduct OT evaluations and periodic reassessments; identify barriers to safe self‑care.
Develop and implement individualized OT interventions (self‑care training, HEPs, adaptive equipment training, cognitive strategies, UE rehab).
Recommend home/environmental modifications and DME as appropriate.
Educate caregivers on safe transfers, energy conservation, and routines.
Document thoroughly in EMR; adhere to Medicare CoPs and payer rules.
