Role Overview

Evaluate mobility and functional deficits in the home, design evidence‑based treatment plans, and restore safe independence. Educate patients/caregivers and coordinate with nursing and rehab colleagues.

Key Responsibilities

Perform PT evaluations and reassessments; set measurable, functional goals.
Deliver therapeutic exercise, gait/balance training, neuromuscular re‑ed, home‑safety modifications, and DME recommendations.
Create/update the PT plan of care and collaborate on the interdisciplinary plan.
Educate on home exercise programs, fall prevention, and energy conservation.
Document timely/accurately in EMR and ensure regulatory/payer compliance.
